Ok, the picture does not show things very well shot from the front. I just now see that it is a hole, at first look it totally looked flat.
Yeah, probably a caterpillar kind of thing. Not sure if you have them there, but tomato moths are quite common around here, and their caterpillars make similar looking holes in the tomato (usually near the stem end however).
