Sorry to bother you all but can you help me?

Question one: What disease do I have on this leaf and do you know the name of this tomato of the first two pictures?

Question two: I know its not a disease question but since you are helping with question one. I figure I can ask question two here as well. Let me know if that's okay if not, then direct me to where I can post.
